It is patient-centered, empowering the individual to be the driving force in their own recovery.
The Behavioral Health Home (BHH) is an effective healthcare service delivery model to promote whole person health. It focuses on the integration of primary care, mental health, and social services to support adults and children diagnosed with mental illness. The BHH model of care uses a multidisciplinary team to coordinate and deliver person-centered services designed to support an individual in reaching their health and wellness goals.
As a participating BHH provider, you can find the tools, methods and resources you need to support the BHH model on this website. This can help you improve patients’ access, outcomes, quality of treatment, and experience.
